5 Day Class - $1850, or monthly payments of $206 (Mon-Fri 12:00 to 2:30)

3 Day Class - Morning (Mon, Wed, Fri 9 - 11:30 am) - $1050, or monthly payments of $117

3 Day Class - Afternoon (Tue, Wed, Thurs 12 - 2:30 pm) - $1050, or monthly payments of $117

2 Day Class - $850, or monthly payments of $95 (Tue, Thurs 9 to 11:30 am)

Authorization for Emergency Medical Contact

In case of emergency or serious illness, when parents cannot be reached immediately, I hereby authorize the person in charge to obtain emergency medical care and/or provide emergency medical transportation for my child. I give consent for necessary treatment when my child is in the care of a physician and/or hospital/clinic.

Photography Release

Optional. My child may be photographed for preschool purposes including use of the website.

Waiver and Release of All Claims

As a parent/guardian of a participant in this program, I recognize and acknowledge that there are certain risks involved and I voluntarily agree to assume the full risk of any injuries, including damages or loss, regardless of the severity, that my minor/child/ward may sustain as a result of participating in any and all activities connected with or associated with such activities held on or off the property. I waive and fully release Discovery Adventures Preschool from any and all claims as a result of participating in this program.

Due to high demand, enrollment forms will be reviewed and you will be notified whether I am able to accept your application. If we are unable to enroll your child, the enrollment fee will be fully refunded. (Once your child is enrolled the enrollment fee is now non-refundable). Priority will be given to students who are re-enrolling for a second year. Next priority will be to students who have an older sibling I have taught in previous years. If there is space available for new families, I will start with who enrolled fist.
